Dr. Susan Jacob has practiced family medicine in Zionsville for over 25 years, building long-lasting, multigenerational relationships with the patients and families she serves. As a graduate of Indiana University School of Medicine and former Chief Resident at St. Vincent’s Family Medicine Residency, Dr. Jacob is Board Certified by the American Board of Family Medicine and has dedicated her career to providing comprehensive, compassionate care across all stages of life, from newborns to seniors. As part of FreedomDoc Zionsville, she brings her deep commitment to relationship-based care to a direct primary care (DPC) model, allowing her to deliver the personalized, timely care patients deserve.

Dr. Jacob’s approach to medicine centers on accessibility, understanding, and continuity. Members of her direct care practice enjoy enhanced communication through phone, text, or video chats directly with Dr. Jacob. She emphasizes the importance of working together to find the best solutions for you and your health, from making lifestyle changes to managing chronic conditions. If you require care outside the practice, she will coordinate with local specialists, drawing on decades-long healthcare relationships in Zionsville and the north side of Indianapolis. Her deep roots in the local community and extensive healthcare experience make her a trusted partner in you and your family’s health journey.

Dr. Jacob enjoys spending time with her husband, Jeff, and their two sons in her spare time. Her hobbies include hiking, camping, and managing their small family farm.  As a family, they enjoy traveling, especially vacationing at their family cabin in Northern Ontario.
